Coach Action Plan for Suspected Concussion
If you suspect that a player has a concussion, you should take the following steps:
- Remove the athlete from play.
- Fill out required WAHA reporting form.
- Ensure the athlete is evaluated by an appropriate health care professional. Do not try to judge the seriousness of the injury yourself.
- Inform athlete's parents or guardians about the possible concussion and provide them with the Concussion Info Sheet for guardians (found in the Useful Links section) and the web links to USA Hockey's concussion information.
- Promptly inform the player's Age Level Director.
- Fill out the top portion of the USA Hockey Return to Play form and share with parents or guardians of the athlete (found in the Useful Links section).
- Allow athlete to return to play only with permission from an appropriate health care professional and completed Return to Play form.

WAHA Concussion Notes
The USA Hockey/WAHA Concussion Management Protocol and the state statutes require that an athlete be removed from any training, practice or game if they exhibit any signs, symptoms or behaviors consistent with a concussion or are suspected of sustaining a concussion. The player should not return to physical activity until he or she has been evaluated by a qualified medical provider who has provided written clearance to return to sports.

Concussion Signs & Symptoms
Signs Observed by Coaching Staff:
- Appears dazed or stunned
- Is confused about assignment or position
- Forgets sports plays
- Is unsure of game, score, or opponent
- Moves clumsily
- Answers questions slowly
- Loses consciousness (even briefly)
- Shows behavior or personality changes
- Can't recall events prior to hit or fall
- Can't recall events after hit or fall
Symptoms Reported by Athlete:
- Headache or "pressure" in head
- Nausea or vomiting
- Balance problems or dizziness
- Double or blurry vision
- Sensitivity to light
- Sensitivity to noise
- Feeling sluggish, hazy, foggy, or groggy
- Concentration or memory problems
- Confusion
- Does not "feel right"
